Changes to self-assessment forms

We have made it simpler to apply for support online. Our needs assessment covers all areas of social care, including equipment, adaptations and sensory loss.

We have removed the ‘equipment, adaptations and mobility assessment’ and ‘sensory assessment’. If you have already started these forms, you can still submit them until 8 June.

Carers can still complete a carers assessment separately.

Registering for an account

To get full access of the Portal, you need to register for an account. This allows you to save, print, download and submit your details.
It's especially important if you need to save a form and come back to it later.
To register:
1. You'll need a valid email address that you use regularly.
2. Enter this email address and create a password that fulfils the policy.
3. We'll email a verification code to the email address you used to register.
4. You will need to enter a verification code to register and then each time you log in.
5. The code is only valid for 20 minutes. Once it expires, you need to request a new one.
6. If you don't receive the verification code, check your Junk folder.
