What is the Adults Portal?
The Adults Portal is a web-based system available 24 hours a day. It enables people who live in East Sussex to complete and submit forms to Adult Social Care online. Family members, friends and professionals can also apply on behalf of someone else.

Needs assessment
The needs assessment form is used to apply for social care support. For example, help with personal care, housework and getting out and about.
Equipment, adaptations and mobility assessment
This form is for people who struggle to use or get around their home and would benefit from equipment or adaptations. This includes things like grab rails, perching stools and raised toilet seats.
Report a safeguarding concern
You can use this form to tell us about an adult who is at risk of abuse, harm or neglect. If the person is in immediate danger, please call the police on 999 instead.
Carer's assessment and emergency plan
If you are an unpaid carer, you can submit a carer's assessment to find out if you are eligible for social care support.
You can also register an emergency plan to make sure that short-term cover can be put in place in an emergency.
Financial eligibility checker
This form asks some simple questions about your finances. It will tell you if you are likely to pay towards the cost of your care.
