Changes to self-assessment forms

We have made it simpler to apply for support online. Our needs assessment covers all areas of social care, including equipment, adaptations and sensory loss.

We have removed the ‘equipment, adaptations and mobility assessment’ and ‘sensory assessment’. If you have already started these forms, you can still submit them until 8 June.

Carers can still complete a carers assessment separately.

As a carer, you're entitled to an assessment to find out how caring impacts on your life. You can tell us about your needs using the carer's assessment form.

Register a carer's respite emergency plan

Register a carer's respite emergency plan
You can set up an emergency plan to give you peace of mind. This allows you register three people who can help if you are unable to carry out your caring role due to an emergency. We will contact these people on your behalf.
