Carer's respite emergency plan
Fill in this form to register an emergency plan with the Carer's Respite Emergency Support Scheme (CRESS). This will ensure that we can arrange short-term cover in an emergency.
You can set up an emergency plan to give you peace of mind. This allows you register three people who can help if you are unable to carry out your caring role due to an emergency. We will contact these people on your behalf.

How it works
This form will ask for contact details of up to three people who could support the person you care for.
You'll be given an East Sussex Carers Card to carry with you. In an emergency, you or someone else can call the number on the card. Adult Social Care will then contact the people in your plan. This service is available 24 hours a day, 7 days a week.
If none of the people listed in your plan are available, we'll contact you or a person on your behalf to find out what support is needed.
Completing the form
Please read these notes before you start
- It will take up to 1 hour to complete the form.
- You'll need to register using an email address. This will allow you to save your form and come back to it at any time.
- For security reasons if there has been no activity for 30 minutes your session will expire. You can save your form at any point using the Save for later button.
- Complete the form as accurately and comprehensively as possible. The form will guide you to additional questions where appropriate.
Make sure you have the following to hand
- Your email address. You cannot submit a form without it.
- Details of the person you care for: name, address and date of birth.
- Details of up to 3 people who could support the person you care for.
