Reporting a concern

If you're concerned that you, or someone you know about, may be being abused, neglected or exploited, please tell us.

If the person is in immediate danger, contact the emergency services on 999.

If you're submitting this form about someone else, ideally you should discuss this with them and seek their consent, explaining that one possible outcome may be a safeguarding enquiry. But you don't need someone's consent to raise a safeguarding concern.

We'll email you in response to this form. If you're worried that receiving emails could put you or the person you're concerned about at risk, phone us on 0345 60 80 191 (open 8am to 8pm 7 days a week including bank holidays).

How long will it take?

The form takes approximately 20 minutes depending on the amount of information you need to provide. For security reasons, your session will timeout after 30 minutes of inactivity. You can save the form at any point using the 'save for later' button at the bottom of the screen. We'll email you a link and the form will be available for 30 days in your list of saved forms.

Will I have to register?

You'll need to register using an email address. This allows you to save your form and come back to it at any time.

• The person you are referring
• Your details
• Details of alleged abuse
• Your concerns

What happens next?

We'll email you to say we have received this form. You'll receive another email to update you when we have reviewed the information you've provided.
