Reporting a concern
If you're concerned that you, or someone you know about, may be being abused, neglected or exploited, please tell us.
If the person is in immediate danger, phone the police on 999.
If you're submitting this form about someone else, ideally you should discuss this with them and seek their consent, explaining that one possible outcome may be a safeguarding enquiry. But you don't need someone's consent to raise a safeguarding concern.

How long will it take?
The form takes approximately 20 minutes depending on the amount of information you need to provide. For security reasons, your session will timeout after 30 minutes of inactivity. You can save the form at any point using the 'save for later' button at the bottom of the screen. We'll email you a link and the form will be available for 30 days in your list of saved forms.
Will I have to register?
You'll need to register using an email address. This allows you to save your form and come back to it at any time.
