Direct Payment contingency plan information

There is a chance that any paid or unpaid care arrangements you have in place could break down due to sickness or self-isolation. It is important that you and your carers are prepared if this happens.
 
This form is used to plan for any situation where your care may break down.

• The form will take about 15 minutes to complete.
• You'll need to register using an email address. This will allow you to save your form and come back to it at any time.
• Once completed download and keep this contingency plan with your care and support plan, if you have one, so that you can easily refer to it.
• Remember to update your contingency plan if any of the details change.

Next steps

Once you have submitted the form, you will receive an email confirmation. Your plan will then be added to your social care record for future reference.
