A further authorisation is a new request for the same person. When an existing DoLS Authorisation is coming to an end the managing authority must review whether it is still necessary. It is possible, at any stage, that things have changed and the person no longer needs such a restrictive environment. In this case the managing authority needs to request a review to cease the DoLS authorisation (Form 9 – standard authorisation ceased).
If however, having reviewed the person's current situation, the managing authority concludes that the authorisation needs to continue then you will need to request a further authorisation . You can apply up to 28 days in advance and should not leave this to the last moment.
Before you start
Before you complete the online authorisation you must inform the person's family that you have made an application for DOLS authorisation.
The form will take 15-20 minutes to complete depending on the amount of information you need to provide.

Details of the person you care for: name, address, and date of birth.
Details of anyone else you wish to be involve in your assessment.
