Deprivation of Liberty Safeguards (DoLS) form 10

Request a review

To request a review of an existing DoLS referral you require a DoLS form 10. The review may arise from one of the following:

A request received to review an existing standard authorisation; or

Information received that may indicate requirement for a review

Before you start

The form will take 15-20 minutes to complete depending on the amount of information you need to provide.

You'll need to register using an email address. This will allow you to save your form and come back to it at any time.

When you encounter a question with a red asterisk (*), it means the question is mandatory and must be answered to continue.

Make sure you have the following to hand

• Details of the person: name, address, and date of birth.
• Details of anyone else you wish to be involved with your assessment.
