Deprivation of Liberty Safeguards (DoLS) form 1 - standard authorisation

In most cases it should be possible to apply for a standard authorisation before the need for deprivation of liberty begins.

Before you start

Before you complete the online authorisation you must inform the person's family that you have made an application for DOLS authorisation.

The form will take 15-30 minutes to complete depending on the amount of information you need to provide.

You'll need to register using an email address. This will allow you to save your form and come back to it at any time.

When you encounter a question with a red asterisk (*), it means the question is mandatory and must be answered to continue.

Make sure you have the following to hand

• Details of the client: name, address, and date of birth.
• Details of anyone else you wish to be involves in the assessment.
