Deprivation of Liberty Safeguards (DoLS) - Submit new information

If you are a Care Home or Hospital and have submitted a request for a Deprivation of Liberty Safeguards assessment. Please read the following:

East Sussex County Council is experiencing a huge increase in requests for assessments as a result of the Cheshire West Judgment. For this reason referrals are first triaged to determine their priority. This means assessments are not undertaken in the order of there receival date

If you have any information not already supplied with your DOLS referral that suggests a higher priority than normal. Please provide this information as soon as possible.

Before you start

• Before you complete the online authorisation you must inform the person's family that you have made an application for DOLS authorisation.
• The form will take 15-30 minutes to complete depending on the amount of information you need to provide.
• You'll need to register using an email address. This will allow you to save your form and come back to it at any time.
• When you encounter a question with a red asterisk (*), it means the question is mandatory and must be answered to continue.

Make sure you have the following to hand

• Details of the adult: name, address, and date of birth.
• Details of anyone else you wish to be involves in the assessment.
