Deprivation of Liberty Safeguards (DoLS) - Monitoring conditions

The purpose of this form is to help Managing Authorities meet their statutory duties. Monitoring any standard authorisation which is in force for any person in their care.

MCA DOLS Code of Practice 2008 – Chapter 8

When a persons deprived of their liberty, the managing authority has a duty to monitor the case on an ongoing basis. If the person's circumstances change the person may no longer need depriving of their liberty. Or it would make it appropriate to amend, delete an existing condition or add a new condition.

Before you start

  • Before you complete the online authorisation you must inform the person's family that you have made an application for DOLS authorisation.
  • The form will take 15-30 minutes to complete depending on the amount of information you need to provide.
  • You'll need to register using an email address. This will allow you to save your form and come back to it at any time.
  • When you encounter a question with a red asterisk (*), it means the question is mandatory and must be answered to continue.

Make sure you have the following to hand

  • Details of the adult: name, address, and date of birth.
  • Details of anyone else you wish to be involves in the assessment.
