How long will it take?

The needs checker will take you about 5 minutes to complete.

Will I have to register?

You don't have to register or provide personal details for it.

What do I need to tell you?

We will ask you a few simple questions about your health conditions and/or disabilities and how they affect your daily life. This checker will give you an indication as to whether your needs are likely to be eligible for support from Adult Social Care.

What happens next?

Once completed, you can decide whether you want to: find your own care and support services; complete a financial checker; or register to complete a detailed needs assessment and submit this to us.

How long will it take?

This needs assessment will take you about 15 minutes to complete.

Will I have to register?

You will need to register as this enables you to save your form so you can come back to it at any time. You will need an email address in order to register.  

What do I need to tell you?

This is a more detailed form to start the assessment process. It will help us to work with you to find out what support you might need. We will ask more detailed questions about your care needs and you will have free text boxes to supply details.

What happens next?

Once completed, we will ask you to complete a financial calculator to provide details of your finances. This will calculate what you may have to pay towards the cost of care that is arranged by the local authority. This financial calculator will take you about 15 minutes to complete.
