Changes to self-assessment forms

We have made it simpler to apply for support online. Our needs assessment covers all areas of social care, including equipment, adaptations and sensory loss.

We have removed the ‘equipment, adaptations and mobility assessment’ and ‘sensory assessment’. If you have already started these forms, you can still submit them until 8 June.

Carers can still complete a carers assessment separately.

Your form has been submitted to Adult Social Care

We will send you an email to confirm we have received it.

You can access all forms you submit to us by logging on to the Adults Portal.

These forms are accessible to you for 90 days from submission, after which they will be automatically deleted.

We will review your form. If we need to update you this will either be by phone, email or via the Adults Portal.

If you have not heard anything you can phone us on 0345 60 80 191 (open 8am to 8pm 7 days a week including bank holidays).
